Making Care Affordable
Each year, King’s Daughters provides assistance to thousands of patients
who are struggling financially to afford healthcare.
If you are unable to afford your care, please contact our Financial Counselors
as soon as possible so that we can help you sign up for government programs
that can help, identify resources to assist, walk you through setting
up payment plans, or possibly reduce your financial obligation.
For those who qualify, we may be able to write off as much as 100 percent
of your bill, based upon the U.S. Department of Health and Human Services
Poverty Guidelines.
To begin the process, please complete the
Financial Assistance Determination Form, which is available by calling (606) 408-4118.
Once completed, you may submit your application by:
- Dropping it off at the Cashier’s Office located in the main Registration
Department on the first floor of the Medical Center
- Mailing it to us in the envelope provided
-
By email to:
financialassistanceteam@kdmc.net
- Faxing it to us at (606) 408-6917
If you have any questions about the form or the information required, please
call: (606) 408-4118 or toll free 1-866-408-6466. Once we have received
and reviewed your application, a member of our Team will contact you for
additional information or clarification.
