Your Records Where & When You Want Them
Your medical record is maintained electronically at King’s Daughters. We use an electronic medical record (EMR) system called Epic, which allows King's Daughters-associated providers to access your medical records wherever you are.
The Epic system integrates information from anywhere you receive treatment at King’s Daughters. It allows physicians and caregivers to have a complete clinical profile, in a secure, state-of-the-art system.
As a patient of King's Daughters, you have free secure online access to portions of your medical record. Access is secure and completely free.

To sign up for MyChart, contact your physician’s office and request your special access letter. The letter will provide you with a password and instructions on setting up your MyChart account.
Copies of Medical Records
The Medical Records Department and the Film Library are currently closed to the public due to the COVID-19 pandemic. Please call our Medical Records department at (606) 408-1815 to make arrangements to receive your medical records or the film library at (606) 408-0746. Records can be mailed via U.S. Mail; via email; or sent electronically to your MyChart account.
For your convenience, we have provided a PDF of our ACCESS FORM, which will need to be completed to arrange for release of your records. You may mail, fax, or email the form to us, along with a copy of your state-issued photo ID (such as a drivers' license). Our fax number is (606) 408-6797. Emails can be sent to medicalrecords@kdmc.kdhs.us.
Depending on the records/imaging studies you are requesting, it may take up to 21 days to process your request. Please be as specific as possible when completing the Access Form. If you need your imaging studies supplied on a disc, please note this on the form.

Retrieving your OLBH medical records
To obtain copies of your Our Lady of Bellefonte Hospital medical records, please contact CIOX at 1-866-625-7130. King's Daughters does not have copies of patients' OLBH records unless the patient has made specific arrangements to have the records transferred to us.
To obtain copies of OLBH radiology films, please contact the Bon Secours Radiology Imaging Resource Center at (804) 287-7624. Hours are 7:30 a.m. to 5 p.m. Monday - Friday. Requesting facilities must fax a proper Release of Information form to (804) 287-7584.
Following a request, the Radiology Imaging Resource Center will send the images to King’s Daughters. The images will be uploaded to our PACS system for continuation of care. If the patient requests images to be sent to their address, the Radiology Imaging Resource Center will deliver/mail the image disk to the patient and the patient will need to give these to their provider directly.
