MyChart

Your Records Where & When You Want Them

Your medical record is maintained electronically at King’s Daughters. We use an electronic medical record (EMR) system called Epic, which allows King's Daughters-associated providers to access your medical records wherever you are.

The Epic system integrates information from anywhere you receive treatment at King’s Daughters. It allows physicians and caregivers to have a complete clinical profile, in a secure, state-of-the-art system.

As a patient of King's Daughters, you have free secure online access to portions of your medical record. Access is secure and completely free.

Benefits of MyChart include:

  • Request prescription renewals
  • Get test results
  • View your radiology images
  • Keep track of your health, including current issues, medications, growth charts, allergies, immunizations and medical, surgical and family history.

To sign up for MyChart, contact your physician’s office and request your special access letter. The letter will provide you with a password and instructions on setting up your MyChart account.

Copies of Medical Records

The Medical Records Department is located on the 2nd floor of King's Daughters Pavilion, 2000 Ashland Drive, Russell. Patients may request copies of their medical records in person, by phone to (606) 408-1815 or electronically through their MyChart account. Requests for imaging records should be directed to the film library at (606) 408-0746. Records can be picked up in person at Medical Records, mailed via U.S. Mail; sent via email; or sent electronically to your MyChart account.

For your convenience, we have provided a PDF of our ACCESS FORM, which will need to be completed to arrange for release of your records. You may bring, mail, fax, or email the form to us, along with a copy of your state-issued photo ID (such as a drivers' license). Our fax number is (606) 408-6797. Emails can be sent to medicalrecords@kdmc.kdhs.us.

Depending on the records/imaging studies you are requesting, it may take up to 21 days to process your request. Please be as specific as possible when completing the Access Form. If you need your imaging studies supplied on a disc, please note this on the form.
