Your Records Where & When You Want Them
Your medical record is maintained electronically at King’s Daughters.
We use an electronic medical record (EMR) system called Epic, which allows
King's Daughters-associated providers to access your medical records
wherever you are.
The Epic system integrates information from anywhere you receive treatment
at King’s Daughters. It allows physicians and caregivers to have
a complete clinical profile, in a secure, state-of-the-art system.
As a patient of King's Daughters, you have free secure online access
to portions of your medical record. Access is secure and completely free.
Benefits of MyChart include:
- Send an appointment request to your provider 24/7/365
- Request prescription renewals
- Get test results
- Keep track of your health, including current issues, medications, growth
charts, allergies, immunizations and medical, surgical and family history.
To sign up for MyChart, contact your physician’s office and request
your special access letter. The letter will provide you with a password
and instructions on setting up your MyChart account.
Copies of Medical Records
Imaging Studies: For copies of CT, MRI or other imaging studies, please call the KDMC Film
Library at (606) 408-0746 at least 24 hours in advance. You will be asked
to complete a release form and to bring a valid drivers license or other
form of state-issued ID to pick up the records.
Medical Records: Please call our Medical Records Department at 606-408-1815. The department
is open from 8 a.m. to 5 p.m. Monday through Friday. You will need to
complete a Consent to Release Information form. Depending upon the records
you are requesting, it may take up to 21 days to process your request.
