Your Records Where & When You Want Them
Your medical record is maintained electronically at King’s Daughters.
We use an electronic medical record (EMR) system called Epic, which allows
King's Daughters-associated providers to access your medical records
wherever you are.
The Epic system integrates information from anywhere you receive treatment
at King’s Daughters. It allows physicians and caregivers to have
a complete clinical profile, in a secure, state-of-the-art system.
As a patient of King's Daughters, you have free secure online access
to portions of your medical record. Access is secure and completely free.
Benefits of MyChart include:
- Send an appointment request to your provider 24/7/365
- Request prescription renewals
- Get test results
- Keep track of your health, including current issues, medications, growth
charts, allergies, immunizations and medical, surgical and family history.
To sign up for MyChart, contact your physician’s office and request
your special access letter. The letter will provide you with a password
and instructions on setting up your MyChart account.
Copies of Medical Records
The Medical Records Department and the Film Library are open 8 a.m. to
5 p.m. Monday through Friday. We are located on the first floor of the
Lexington Avenue Parking Garage. You can reach us at (606) 408-1815.
To obtain a copy of your medical records and/or imaging studies, please
complete the ACCESS FORM (link below) and bring it with you to the Medical
Records department along with a valid photo ID. For your convenience,
you may also fax the form along with your photo ID to (606) 408-6797 or
scan and email the documents to
Depending on the records/imaging studies you are requesting, it may take
up to 21 days to process your request. Please be as specific as possible
when completing the Access Form. If you need your imaging studies supplied
on a disc, please note this on the form.
