Coumadin Clinic

During your initial visit to one of the Coumadin Clinics, the pharmacist will spend about 30 minutes with you reviewing your medical history, discussing the risks, benefits and purposes of anticoagulant therapy and answering your questions. After this initial visit, your appointments will last about 15 minutes. Depending upon the location where you are being seen, you will either meet with the pharmacist in person, or through a telehealth visit, which takes place over a secure, private internet connection.

The Coumadin Clinic is beneficial to people being treated for heart conditions, such as:

  • atrial fibrillation (Afib) or valve replacement
  • pulmonary embolism (PE) or deep vein thrombosis (DVT)
  • certain vascular conditions
  • or those with a genetic disorder that causes hypercoagulability, such as Factor V Leiden.

Patients are seen by physician/provider referral, typically within seven days of referral. Newly diagnosed patients will be seen weekly. As international normalized ratio (INR) levels stabilize, patients are seen less often. The typical appointment interval, once stabilized, is six weeks.

We work with you and your physician in managing your anticoagulant therapy. Maintaining appropriate PT/INR numbers is essential to your health and safety. Our clinics work in partnership with your physician/provider to ensure your anticoagulant therapy is as safe and effective as possible. Lab draws, international normalized ratio testing and medication adjustments all occur at the same visit, saving you time and ensuring the most up-to-date lab values are used in monitoring treatment success.
