The Admissions Process

A member of our admissions team will meet with you or your caregiver during the admissions process to gather important information. During this time you or your guardian/POA will be required to fill out a short admissions agreement. Office hours are 8 a.m. to 4:30 p.m. Monday through Friday. It is important that you provide us with the following documentation prior to or during your admission to Kingsbrook:

  • Medicare Card
  • Medicaid Card
  • Any other healthcare insurance card
  • Social Security Card
  • Advanced Directives, Living Will, Power of Attorney ( If Applicable )
  • Power of Attorney Documents ( Clinical & Financial)
  • Long-Term care/supplemental insurance policies

Our admissions team will review necessary state and federal required documentation as well as our admissions agreement and provide you with a copy.
