Your situation is unique and its important you make an informed decision about your health that takes into account your needs and personal preferences. We have outlined the criteria for admission, the process involved in admission, as well as some frequently asked questions. The Admissions Process

One of our Admissions Coordinators will meet with you or your caregiver during the admissions process to gather important information. During this time you or your guardian/POA will be required to fill a short admissions agreement. Office hours are 8 a.m. to 4:30 p.m. Monday through Friday. It is important that you provide us with the following documentation prior to or during your admission to Kingsbrook:

  • Medicare Card
  • Medicaid Card
  • Any other healthcare insurance card
  • Social Security Card
  • Advanced Directives, Living Will, Power of Attorney ( If Applicable )
  • Power of Attorney Documents ( Clinical & Financial)
  • Long-Term care/supplemental insurance policies

One of our admissions coordinators will review necessary state and federal required documentation as well as our admissions agreement and provide you with a copy.
